Michael Colotta is a board-certified Family Nurse Practitioner. He obtained his Bachelor’s
degree in nursing from Stephen F. Austin State University and his Master of Science in Nursing from the University of Texas at Tyler.

Michael has been practicing at Henderson Family Medicine since 2011.

Michael began his nursing journey in Houston, Texas and then moved to Nacogdoches in 1996 to continue his education at Stephen F Austin. His background includes Orthopedic Medicine, Med-Surg, Emergency Medicine, and Interventional Radiology. His scope of practice encompasses integrative wellness, health promotion, disease prevention, and management of common and complex healthcare problems

He believes in building long-lasting and trusting relationships with patients and their
families. He seeks to get to know patients by taking the time to listen
and understand their particular needs and to help them make informed decisions
regarding their health, as well as the health of their loved ones.

When Michael is not in his healthcare role he enjoys fishing, hunting, and spending time with his wife Carey and their four kids.

Dyllon Sanford is a proud resident of Henderson. Texas. He graduated with his Bachelor of Science in Exercise Physiology from Texas A&M University in 2015. He completed his Master of Physician Assistant Studies at Texas Tech University Health Sciences Center in 2018.

Dyllon has been with Henderson Family Medicine since 2021.

His experience includes Rural Medicine, Emergency Medicine, Family Medicine, and Orthopedics.

He wants his career to be defined by his compassion, intentionality with patients, and excellent care.

When he isn't providing care to his patients, you can find him spending quality time with his wife and two sons.

Alicia Juarez, MSN, APRN, FNP-C

Tad Titlow is a Family Nurse Practitioner board-certified by the American Academy of Nurse Practitioners. He is a native of the East Texas area, born and raised in the small community of Tennessee Colony, Texas, just outside of Palestine. He obtained his bachelor’s degree in nursing from the University of Texas and his Master's Degree in Nursing from Texas Tech University at Lubbock, Texas. His scope of practice encompasses health promotion, disease prevention, diagnosis, and management of common and complex healthcare problems beginning in childhood and continuing throughout the aging process.

He began his career in healthcare by working at East Texas Medical Center Emergency Department for 5 years then pursuing his degree as a nurse practitioner. Tad has worked in several settings as a nurse practitioner including Emergency, Rural Health Center, and Private practice. He strives to build a long trusting relationship with his patients and their families. Tad seeks to acquire a comprehensive evaluation by listening to and understanding their needs.

When Tad is not in his healthcare role he enjoys spending time with his wife Linda and their kids, hunting and horse riding. Being raised on a dairy farm, Tad understands and adheres to hard work when he is either at home or at work. His most important priority is his Savior, Jesus Christ who saves all who call upon Him.

Alicia Juarez is a lifetime resident of Nacogdoches, Texas, and the newest edition to Henderson Family Medicine. She is a board-certified Family Nurse Practitioner with over 13 years of Emergency Medicine and Med-Surg experience.

Alicia obtained her Bachelor of Science in Nursing at Stephen F. Austin State University in 2010. She went on to obtain her Masters of Nursing and Nurse-Practioner license at Stephen F. Austin as well.

When Ali isn't providing care to her patients she enjoys spending quality time with her husband and 2 daughters.
